You can enroll with FHIS without coming to our office through this site. For you to enroll with us through this site, you are required to send us the following information either through email or sms message:
After you have given us these information, we will generate a PIN which you will use to fill our online form. We advise you keep a copy of the information you send to us because your PIN will be tied to the exact (eg.spelling) information you sent. What this means is that, if you sent us your firstname as "Peter", and we generate PIN based on this, on validating your PIN, if you enter "Peters" as your firstname, our software will reject your PIN.
